Write sick leave notes, medical reports, fitness certificates and attendance letters, signed and printed on your letterhead.

1Write a report

Open the tab
Open the patient’s visit (or their record) and click the “Reports & certificates” tab.

Choose the type
Sick leave note (first and last day off; the number of days is counted for you), medical report (for an employer, insurer, school, court or another doctor), fitness certificate (fit, fit with restrictions or not fit, and for what) or attendance letter.

Write it
Fill in who it is addressed to, the subject and the body. Choose whether the diagnosis is shown: leave it off unless the patient agrees.

Save or sign
“Save draft” keeps it editable by you. “Sign & print” finalises it with your name, cadre and licence number and opens the printout on your letterhead.

Corrections
A signed report cannot be edited. Use “Add addendum” to add a dated correction or update; it prints with the report.

Void
If a report was issued in error, use “Void report” with a reason. It stays on the record marked void and cannot be reprinted as valid.
